Where do I find a spending account claim form?
The Spending Account Reimbursement Claim Form is submitted when you need to be reimbursed for an eligible claim expense.
To locate the form, you can do so via your MyHumana account. Once signed in, scroll to your spending accounts, and click the account you are seeking reimbursement from. (Example: HSA) On the right hand side of your screen, you will see a link you can click to "Request Reimbursement", as well as "Tools and Forms" that you can choose from.
