When it comes to wisdom teeth removal - benefits are first processed under your medical plan when the tooth/teeth are impacted (below the gumline), then processed to your dental plan for secondary benefits.
If the tooth/teeth are erupted (above the gumline) this would be strictly processed to your dental plan. (your medical plan benefits do not offer coverage here).
According to your medical plan benefits, when the provider is in network, eligible services are payable at an 80% coinsurance level following your $1500 deductible. There is a maximum out of pocket of $4500.
