How do I file a claim for reimbursement for out of network charges?

I recently found a provider on this website for an annual eye exam. I made the appointment and when I went the eye doctor's office said they do not accept Humana Insight Network because the doctor is an Opthomologist and is considered a specialist. They take Human Insight Network for eyewear. I called and spoke to a Humana Insight Network who verified that in your system this doctor did not specify that on the provider list. The representative went ahead and said to pay with the medical card and that I should file a claim with Humana for reimbursement of $30 from the $50 I had to pay.  I would like to file a claim to get reimbursed.
