Do you offer coverage for Botox injections used to treat migraines caused by TMJ?

I have exhausted all treatment options for my TMJ symptoms and the weekly migraines experienced as a result of having it. I tried Botox injections several years ago and paid out of pocket because my insurance at the time didn’t cover it. The Botox treatment was effective but extremely expensive and I can’t afford it, even though it’s the only treatment I’ve found that works. I was wondering if I have coverage for using Botox injections as a treatment for chronic migraines caused by TMJ? If not, are there any waivers or forms available that my doctor can fill out proving I have exhausted all other treatment options and am in need of this treatment, and should receive coverage?
