Before the schedule the implant and purchase of the device, I need to know what the out-of-pocket costs will be.
Good day! I would be happy to help!
Here, is our coverage document for this specific service. There are some requirements/items that would have to be met in order to determine eligibility. For that reason, we encourage you to review this with your provider.
You can also view your benefits by following the path given in this thread:
How do I find out my benefit information?
Are there any Humana plans that cover Cochlear Implants or Implantation, osseointegrated implant, temporal bone, with percutaneous attachment to external speech processor/cochlear stimulator? Medicaid/Medicare covers them, why doesn't Humana?
I would suggest reviewing the following with your provider to ensure coverage requirements would be met.
Cochlear, Osseointegrated Implants:

The following codes would NOT be covered:
69710 (Implantation or replacement of electromagnetic bone conduction hearing device in temporal bone)
69714 (Implantation, osseointegrated implant, temporal bone, with percutaneous attachment to external speech processor/cochlear stimulator;)
Humana members may NOT be eligible under the Plan for an intraoral bone conduction hearing device. This is considered experimental/investigational as it is not identified as widely used and generally accepted for the proposed use as reported in nationally recognized peer-reviewed medical literature published in the English language.
